Fill in both sides, including your ticket purchase, contact information, and optional music request. Detach along the dotted line, then mail the registration slip with your payment to the address indicated on the form. Keep the remainder of brochure - it contains valuable schedule and location information.

Please make sure your registration will be received in Cleveland, Ohio by April 30th. No mailed registrations will be accepted after that date but you can pay at the door. All purchases are final and non-refundable. Please keep in mind this is a benefit dance and proceeds are donated to our current beneficiary.
